Patient Forms Prepare Your First VisitYour initial consultation appointment will consist of Dr. Colson reviewing your x-rays, history and discussing the situation to diagnose the problem. You’ll be presented with your treatment options as well as an estimate of your financial responsibility. While there are many procedures we can do the same day, if you wish to have any procedure done under I.V. sedation a consultation with Dr. Colson must be done prior to that appointment date. Important: All patient under the age of 18 must be accompanied by a parent or guardian at the time of all visits to the office.
